You can also dictate without punctuation and use Note Compose to apply it for you. Read the statements below as you would say them normally, without punctuation.
Patient has had a weight loss of 25 pounds over the past 30 days, from 200 to 175 pounds. Patient has been eating 25% of two meals daily, which has reportedly decreased from 50% one month ago.
Now run Note Compose. Tap the Note Compose icon 
above the keyboard, and then tap the Compose Note button.
For best results, say "ONE hundred seventy five" and not "UH hundred seventy five"
