All Topics (General Accuracy)

  • Improved Handling of Prefixes in Dictation – (D-13881; 20251020) In previous topic releases, some words that began with prefixes such as “post” or “mid” were not combined correctly with the root word. For example, “postsurgical” might have appeared incorrectly as “pos tsurgical.” This issue has been fixed, and words like “postsurgical” and “midlateral” now appear correctly as single, combined terms.

  • Improved Recognition of the Word “Hyphen” – (D-13928, 20251020) After the topic release on August 15, 2025, an issue was discovered where saying “hyphen” sometimes still returned the word itself instead of the punctuation mark (-). For example, “C one hyphen bisected shave” was transcribed as “C1 hyphen bisected shave” instead of “C1-bisected shave.” This issue, which occurred when the words before and after “hyphen” were not numbers, was initially addressed in item D-13855 but was later found to persist. We have corrected this behavior again, and “hyphen” now consistently returns the punctuation mark as expected.

  • Improved Handling to Centimeters (cm) – (D-13600, 20251113) We received reports that the word “centimeters” was sometimes transcribed as “CM” or fully spelled out insteTopic Update: 01-05-2026Topic Update: 01-05-2026ad of returning as “cm” in the Pathology topic. Additional handling has been added to ITN to ensure “cm” returns correctly in measurement contexts, and this update now applies across all topics. Lowercase “cm” will return when spoken as part of a measurement, while uppercase “CM” will return only when used in the proper context, such as when dictating “C M” as an abbreviation for case manager.

  • New Drugs Support July 2025 – (S-18472, 20251113) Corpus and dictionary support for new drug names approved by the FDA in July 2025 was added to all topics.

  • July OOVs Added – (S-18578, 20251118) New and previously unrecognized words (a.k.a. “Out of Vocabulary” or OOV) identified in July 2025 AOS corrections have been added to the dictionary and corpus across all topics. Note: Only organizations that allow use of text and audio are included in extracted corrections.

  • August OOVs Added – (S-18728, 20251205) New and previously unrecognized words (a.k.a. “Out of Vocabulary” or OOV) identified in August 2025 AOS corrections have been added to the dictionary and corpus across all topics. Note: Only organizations that allow use of text and audio are included in extracted corrections.

  • Medical Terminology Standardization – (S-19026, 20251226) Support has been added to return the standardized term “heterogeneous” instead of “heterogenous” in dictated content. This update helps ensure medical notes remain accurate, consistent, and aligned with current clinical terminology.

  • New Drug & Device Support August 2025 – (S-18729, 20130213) Corpus and dictionary support for new drug names and medical devices approved by the FDA in August 2025 has been added to all topics.

General Medicine

  • Word Support - (S-18475, 20130213) Support has been added for the following words to help them return more accurately: fructans, Wheat Zoomer, Provitalize, Zepbound, trimethylglycine. and dyspnea

Pathology

  • Improved Issue with Cytokeratin (CK) Stain Formatting -  (D-13999, 20130213) We addressed an issue where abbreviated cytokeratin stain names such as CK5/6, CK7, CK8, and CK20 were occasionally transcribed with an unintended space between the letters and numbers, for example “CK 7” instead of “CK7.” To improve consistency and accuracy, we added missing cytokeratin stain entries to the dictionary, expanded Pathology unigram support for CK stains, removed legacy language model support for approximately 1,000 incorrect CK variants, and implemented substitution rules to help correct any remaining spacing inconsistencies. These updates are intended to improve the accuracy and consistency of cytokeratin stain transcription in pathology documentation.

  • Hyphenation Improvement - (S-19036, 20130213) We introduced a formatting enhancement to support correct and consistent hyphenation of pathology terminology. This update aligns transcription output more closely with established pathology documentation standards.

Radiology

  • 24-Hour Military Time Support – (D-13889, 20251030) Support has been added to ensure 24-hour military times are transcribed correctly. For example, dictating “zero one forty-nine hours central time” now returns “0149 hours central time” instead of “01409 hour central time.”

  • Support Added for “CT head” (D-13931, 20251112) Support has been added for “CT head” to transcribe correctly. Previously, “CT head” could be returned as “CT had.”

  • Support Added for Date Recognition – (D-13910, 20251124) Date interpretation has been refined to reduce occasional misrecognitions, including cases where “eight” was returned as “a” or unexpected words like “toy” appeared. Additional support has been added to help dates transcribe more accurately and consistently, creating smoother and more reliable time entries across workflows.

  • Removed Unsupported Term – (S-18778, 20251124) The term “dol” has been removed from the Radiology topic as requested. It will no longer return in transcribed text, helping keep reports cleaner and aligned with preferred terminology. The term “mag” was requested for removal, but this has been retained for use in phrases such as, “mag citrate,” “MAG technique," or “breast Mag spot.”

  • Support Added for “loop” and “Berylliosis” – (S-18777, 20251201) The word “loop” will now return in lowercase when dictated after “degree,” ensuring consistent formatting in reports. Support has also been added for the term “Berylliosis,” helping transcriptions capture complex clinical language accurately and reliably.

  • “Minor” and “Moderate” Correction – (D-13975, 2025124) An issue has been addressed where the terms “minor” and “moderate” could be interchanged. The system has been updated to help ensure these terms are now used consistently and as intended, helping maintain clarity and accuracy in clinical documentation.

  • “Putting” Returning for “Pudding”  (D-13946, 20251226) Support has been added to improve transcription accuracy for “pudding” consistencies, contrast, and related terminology used in swallow studies. The system now better recognizes and transcribes this term correctly.

  • Improved Accuracy for “x” in Measurements - (D-13952, 20130213) We addressed an issue in which the word “times” was transcribed in place of the symbol “x” when documenting dimensions. For example, “2.3 x 1.3 cm” could normalize to “two point three times one point three centimeters” instead of the expected dimensional format. This update is intended to improve the accuracy and consistency of dimension expressions in clinical documentation.

  • Improved Accuracy for “Millimeters” in STABLE TEXT- (D-13957, 20130213) We addressed an issue where the system could incorrectly recognize the spoken word “millimeters” as “centimeters” in STABLETEXT output. In these cases, the speaker clearly dictated “millimeters,” but the transcription would consistently substitute it with “centimeters.” This update is intended to improve the accuracy and reliability of measurement transcription in clinical documentation.

  • Word Support - (S-18475, 20130213) Support has been added for the following words to help them return more accurately: Pylarify, veiling, and recency.

  • Improved Handling of “Orbit” - (D-14044, 20130213) We updated Radiology topic language support to reduce unintended recognition of the initialism “ORBIT.” This change is intended to improve consistency and ensure that references to “orbit” are handled appropriately in radiology documentation.

  • RADS Terms Added - (D-14060, 20260226) The following standardized reporting terms have been added to the Radiology topic to ensure they are recognized and returned accurately in dictations: BI-RADS, TI-RADS, LI-RADS, L-RADS, NI-RADS, O-RADS, C-RADS, and Bone-RADS. This enhancement improves support for structured radiology reporting terminology and overall dictation accuracy within the Radiology topic.

  • Vocabulary Alignment Update - (S-19164, 20260226) Updates requested for dictionary tokens in the Pathology topic (S-19032) have also been applied to the Radiology language files. These changes include word additions and deletions, as well as updates to spelling, formatting, and pronunciation. This alignment helps improve recognition accuracy and ensures more consistent dictation results in the Radiology topic.

Veterinary

  • Support Added for “uveodermatologic”  – (S-18777, 20251201) Support has been added for the term “uveodermatologic” within the Veterinary topic. This improvement helps ensure your specialized dictation returns accurately, keeping clinical notes clear and precise even with complex terminology.
